This presentation presents some of the best (and next) practices that nurture and sustain student transitions, with a focus on Grades 7-12 (and including college and career readiness). The importance of strong transition teams, professional dialogue, active student leadership, and school-community partnerships will be explored and discussed in-depth. Participants will leave with insights and expertise in current research, and increased knowledge of practical strategies and approaches to transition planning that really works!
